Comprehensive Home Health Solutions is seeking a dedicated Physical Therapist to join our home health team in Northern Nevada (Reno, Sparks, Carson City and surrounding areas). You will evaluate patients in their homes, develop care plans, and guide families through prescribed exercises to restore mobility and independence.

Responsibilities include conducting assessments, collaborating with the interdisciplinary team, and ensuring accurate documentation.

Competenze

• Current Physical Therapist license in Nevada.
• Minimum 1 year of home health PT experience preferred.
• Strong clinical, communication, and problem-solving skills.
• Ability to work independently and manage a flexible caseload.
• Valid driver’s license and reliable transportation.

Main Responsibilities:

Conduct patient assessments, create and update care plans, and establish clear, measurable goals.

Teach patients and their families exercises and self-care techniques that enhance mobility and promote safety.

Complete home safety evaluations and provide recommendations for modifications when necessary.

Work closely with physicians, nurses, and the interdisciplinary team to support high-quality patient outcomes.

Ensure all visit notes and documentation are completed accurately and on time.

Join case conferences and regularly update the care team on patient progress.

Help patients and their families understand care goals, expectations, and ongoing progress.

Qualifications:

Current Physical Therapist (PT) license in Nevada (required).

Minimum 1 year of home health physical therapy experience (preferred).
