Comprehensive Home Health Solutions in Reno, NV is hiring a Physical Therapist (PT) to join our home health team. You will assess patients, develop care plans, and help patients regain mobility and independence in the comfort of their homes.

The role offers flexible scheduling (full-time or part-time), collaboration with a multidisciplinary team, and opportunities for professional development. A Nevada PT license is required, with at least one year of home health experience preferred.

Formación

• Must hold Nevada PT license.
• Minimum 1 year home health PT experience preferred.
• Strong clinical, communication, and problem-solving skills.
• Ability to work independently and manage a flexible caseload.
• Valid driver’s license and reliable transportation.

Responsabilidades

• Assess patients, develop and update care plans with measurable goals.
• Instruct patients and families on exercises and self-care to improve mobility.
• Perform home safety evaluations and suggest modifications.
• Collaborate with physicians, nurses, and the care team to achieve outcomes.
• Complete visit notes and documentation accurately and on time.
• Participate in case conferences and update the care team on progress.
• Explain care goals and ongoing progress to patients and families.
