Job summary

BAYADA Home Health Care in York County is seeking an experienced Physical Therapist (PT) for home health visits across York and Adams Counties. A PA PT license and at least one year of PT experience are required.

Positions available as full-time or part-time on a per-visit basis in patients' homes. The PT will assess, treat, and educate patients, develop and adjust individualized care plans, document observations, and collaborate with a multidisciplinary team to enable safe, independent living

Qualifications

• Must hold a Pennsylvania Physical Therapist license.
• Minimum of one year of PT experience.
• Graduation from a program approved by APTA or equivalent.
• Ability to work independently and manage time effectively.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements related to mobility, gait, balance, and safety.
• Develop and implement individualized care plans including therapeutic exercises.
• Document observations and interventions in EMR with accuracy.
