BAYADA Home Health Care is seeking a qualified Physical Therapist (PT) for PRN, part-time, or full-time home health visits in Lancaster County, PA. A PA PT license and at least one year of PT experience are required.

You will work within a multidisciplinary team to provide rehabilitative care and skilled nursing, helping clients remain at home. Benefits may include a comprehensive plan and 401k; electronic documentation is used for client care management.

Qualifications

• PA PT license required.
• Minimum of one year of recent clinical PT experience.
• Graduation from an APTA-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements (mobility, range, balance, gait, strength, endurance).
• Implement wound care and dressings using evidence-based protocols.
• Develop and implement individualized care plans including manual therapy and gait training.
• Assess and revise the PT care plan with nursing in the multidisciplinary team.
• Educate clients and families to promote health, safety, and independence.
• Accurately document observations and interventions in EMR.
