Job summary

BAYADA Home Health Care, Inc. is seeking a part-time Home Health Physical Therapist to perform outpatient home health visits in the York County area, serving adults and geriatrics in Harrisburg, PA territory. A PA PT license and at least one year of PT experience are required.

Qualifications

• Licensed PA Physical Therapist.
• Minimum of one year of recent PT experience.
• Graduate from a program approved by APTA/AHAA or AMA committee.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make outpatient visits to clients in designated territories.
• Perform diagnostic tests and measurements (mobility, joint range, strength, balance).
• Develop and implement individualized care plans with therapeutic exercises.
• Assess and revise the care plan with nursing as needed.
• Educate clients and family members on rehabilitative care and activities.
• Document observations and interventions using a touch pad tablet.
