Qualifications

• Active Pennsylvania Physical Therapist license.
• Minimum one year licensed PT experience.
• Ability to work independently and manage time.
• Strong interpersonal and problem-solving skills.

Responsibilities

• Make home visits to clients in designated territories.
• Perform mobility and safety assessments and document findings.
• Implement wound care and preventive interventions.
• Develop and adjust individualized care plans with the care team.
• Educate clients and families on rehabilitative activities.
• Document care observations and interventions using a tablet.
