Job summary

BAYADA Home Health Care, Inc. is seeking an experienced Physical Therapist (PT) to provide home health visits in senior living communities across Adams and Franklin counties. The role supports adult and geriatric clients on a per-visit basis.

We offer full-time, part-time, or per-diem opportunities. A PA PT license and at least one year of PT experience are required, with EMR documentation and independent work. BAYADA emphasizes training and career growth.

Qualifications

• Active Pennsylvania PT license required.
• Minimum one year of clinical PT experience.
• Graduation from an APTA-approved PT program as indicated by transcript or diploma.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform mobility, strength, balance, and endurance assessments.
• Implement wound care interventions using evidence-based dressings and topical agents.
• Develop individualized PT plans with manual therapy, gait/balance training, and exercises.
• Continually assess and revise the PT care plan with nursing in the multidisciplinary team.
• Educate clients and family in rehabilitative care to promote health, safety and independence.
• Accurately document observations, interventions, and evaluations in EMR.
