Job summary

BAYADA Home Health Care, Inc. in Carlisle, PA is seeking a licensed Physical Therapist to perform home health visits on a per-visit basis. This full‑time or part‑time role requires at least one year of PT experience and a PA PT license.

Qualifications

• Current Pennsylvania PT license required.
• Minimum of one year of recent PT experience.
• Graduation from a PTA/ABPT program as indicated by transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform mobility, gait, balance and safety assessments.
• Develop and implement individualized care plans with therapeutic exercises.
• Document observations, interventions, and evaluations in EMR.
• Educate clients and family members on rehabilitative care and activities.
