Caring Hand Home Health Solutions, Inc. is seeking a dedicated Physical Therapist to deliver skilled services in patients’ homes within California. The role is per diem, with compensation per visit, and emphasizes independence, mobility restoration, and family-centered care.

The ideal candidate holds a California PT license, has at least 2 years of PT experience (home health a plus), and can adapt to a flexible schedule while prioritizing patient safety and outcomes.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Current CPR certification with AHA.

Responsabilités

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services according to physician’s plan of care and California law.
• Educate patients and families on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and care team; report changes in condition and participate in case conferences.
• Supervise PT assistants and oversee Home Health Aide care as needed.
• Manage documentation including progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals.
