Job summary

Caring Hand Home Health Solutions, Inc. in Grass Valley, CA is seeking a Physical Therapist to deliver skilled home health services.

The per diem role focuses on evaluating mobility, developing treatment plans, and enabling patients to regain independence in their day-to-day activities. The ideal candidate holds a California PT license, a Bachelor's or Master's in Physical Therapy, at least 2 years of PT experience (home health a plus), CPR certification, a valid driver's license and reliable

Qualifications

• Must hold a CA PT license.
• Bachelor's or Master's degree in Physical Therapy.
• Minimum 2 years of PT experience; home health background is a plus.
• Current CPR certification is required.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Evaluate patients' functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with physician's plan of care and California's Physical Therapy Practice Act.
• Educate patients and families on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and care team; report changes in patient condition and participate in case conferences.
• Supervise PTAs and, when PT is sole skilled service, oversee Home Health Aide care bi-weekly.
• Manage documentation including progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.
