Caring Hand Home Health Solutions, Inc is seeking a Physical Therapist to provide skilled home health services in California. You will assess patients, develop individualized plans, and deliver therapy in the home setting, collaborating with physicians and the care team.

Role requires California PT licensure, active CPR, and a flexible schedule. Per diem rate available, with potential to convert to full-time based on performance. Per visit compensation ranges from $160 to $180.

Competenze

• Must hold a California Physical Therapy license (active).
• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Minimum 2 years PT experience; home health a plus.

Mansioni

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver PT services per physician’s plan of care and California PT Practice Act.
• Educate patients and families on exercises, equipment use, and recovery goals.
• Collaborate with physicians and care team; report changes and participate in conferences.
• Supervise PT Assistants and Home Health Aide care as applicable.
• Manage documentation including progress summaries and discharge planning.

Please note that this role is per diem with a rate of $160 - $180 per visit.

Responsibilities

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.

• Deliver physical therapy services in accordance with the physician’s plan of care and California’s Physical Therapy Practice Act.

• Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals.

• Collaborate with physicians and the care team—reporting changes in patient condition and participating in case conferences.

• Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.

• Manage documentation including clinical progress summaries and discharge planning.

• Identify needs for additional home health services and coordinate referrals as appropriate.

• Perform other duties as required for patient services.

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.

• Active California Physical Therapy license.

• Minimum 2 years of PT experience; home health background is a plus.

• Strong communication skills and the ability to connect with patients and families.

• Current CPR certification with AHA.

• Valid driver’s license and reliable, insured vehicle.
