Best Home Health Providers in California is seeking a Physical Therapist for a per diem role. You will evaluate patients’ functional status and mobility to create personalized treatment plans and deliver PT services under physician direction.

Responsibilities include educating patients and families, collaborating with clinicians, supervising aides when needed, and handling documentation. A CA PT license, CPR, and at least 2 years of PT experience—preferably in home health—are required, with a

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Current CPR certification with AHA.
• Valid driver’s license and reliable, insured vehicle.
• Flexible schedule and potential to convert to full-time.

Responsabilidades

• Evaluate patients’ functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with the physician’s plan of care and California’s Physical Therapy Practice Act.
• Educate patients and families on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and care team; report changes in patient condition and participate in case conferences.
• Supervise Physical Therapy Assistants and oversee Home Health Aide care when PT is the sole skilled service.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
