Job summary

Caring Hand Home Health Solutions, Inc is seeking a per diem Physical Therapist to deliver skilled PT in patients' homes across California. You will evaluate function, mobility, and gait; develop individualized plans; educate patients and families; and collaborate with physicians to adjust plans of care.

Requirements include a Bachelor's or Master's in Physical Therapy, active CA PT license, CPR (AHA), valid driver’s license, and minimum 2 years of PT experience; home health is a plus.

Qualifications

• Excellent communication and empathy when interacting with patients and families.
• Minimum 2 years of PT experience; home health background is a plus.
• Active California Physical Therapy license and CPR certification (AHA) required.

Responsibilities

• Evaluate patients’ functional status, mobility, gait, and ROM to develop tailored treatment plans.
• Deliver physical therapy services per physician’s plan of care.
• Educate patients and caregivers on exercises, equipment use, and recovery goals.
• Collaborate with physicians and care team; report changes in patient condition.
• Supervise PT assistants and oversee Home Health Aide care when PT is sole skilled service.
• Complete documentation including progress summaries and discharge planning.
• Coordinate referrals for additional home health services as needed.
