Job summary

Caring Hand Home Health Solutions, Inc is seeking a licensed Physical Therapist to deliver skilled services in patients' homes across California. The per diem role offers flexible scheduling and patient-focused care, emphasizing mobility and independence.

Responsibilities include evaluating function, creating treatment plans, educating families, and coordinating with the care team. A CA PT license, CPR, and a valid driver's license are required, with opportunities to convert to full-time based

Qualifications

• Bachelor's or Master's degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Strong communication skills and the ability to connect with patients and families.
• Current CPR certification with AHA.
• Valid driver's license and reliable, insured vehicle.

Responsibilities

• Evaluate patients' functional status, mobility, muscle strength, gait, and ROM to develop individualized treatment plans.
• Deliver physical therapy services in accordance with the physician's plan of care and California's Physical Therapy Practice Act.
• Educate patients and their families or caregivers on therapeutic exercises, equipment use, and recovery goals.
• Collaborate with physicians and the care team - reporting changes in patient condition and participating in case conferences.
• Supervise Physical Therapy Assistants and, when PT is the sole skilled service, oversee Home Health Aide care bi-weekly.
• Manage documentation including clinical progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals as appropriate.
• Perform other duties as required for patient services.
