Job summary

Caring Hand Home Health Solutions, Inc is seeking a Physical Therapist to deliver skilled services in patients' homes across California. The per diem role focuses on mobility, self-care, and independence, following physician plans and state practice acts.

You will educate patients and families, supervise aides when needed, document progress, and coordinate referrals. A CA PT license, CPR, and reliable vehicle are required; 2+ years of PT experience, preferably in home health, are preferred.

Qualifications

• Bachelor's or Master's degree in Physical Therapy from an accredited program.
• Active California Physical Therapy license.
• Minimum 2 years of PT experience; home health background is a plus.
• Current CPR certification with AHA.
• Valid driver's license and insured vehicle.

Responsibilities

• Evaluate patients' functional status, mobility, muscle strength, gait, and ROM to develop treatment plans.
• Deliver PT services per physician plan of care and California PT Practice Act.
• Educate patients and families on exercises, equipment use, and recovery goals.
• Collaborate with physicians and care team; report changes and participate in case conferences.
• Supervise PT Assistants and oversee Home Health Aide care when PT is sole skilled service.
• Manage documentation including progress summaries and discharge planning.
• Identify needs for additional home health services and coordinate referrals.
