The role focuses on maintaining and improving movement and function, with reexaminations every 30 days and discharge planning.

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from an CAPTE-accredited physical therapy program.
• One year of PT experience in home health care preferred.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Conduct periodic reexaminations (every 30 days at minimum) and adjust treatment.
• Perform discharge planning and other duties as assigned.
