Amedisys is seeking a licensed Physical Therapist to provide home health services. You will diagnose and treat movement limitations, develop care plans, and work to improve patients’ functional mobility in the home setting.

Responsibilities include periodic reexaminations, intervention adjustments, discharge planning, and collaborating with a care team. Prior home health experience and CPR certification are preferred. Mileage reimbursement and full benefits are included.

Qualifications

• Experience in home health care or setting.
• Ability to perform reexaminations at 30-day intervals.
• Experience with discharge planning and patient progression.

Responsabilités

• Help patients maintain, restore and improve movement and function.
• Provide interventions and adjust plans every 30 days as needed.
• Perform discharge planning and other assigned duties.
