Job summary

Vivie is seeking a Physical Therapist (PT) in Home Health to assess, treat, and guide clients back to independence. You'll create personalized plans, lead therapeutic exercises, and collaborate with an interdisciplinary team to improve mobility and quality of life.

This role offers flexible scheduling, pay range of $41.04–$52.00 per hour, and benefits including retirement match, PTO, health coverage, and mileage or car program options. You must hold Minnesota PT license and CPR certification.

Qualifications

• Must be able to read, write, and speak English proficiently.

Responsibilities

• Provide Direct Client Care - Evaluate clients, obtain data, develop personalized treatment plans with short- and long-term goals, deliver high-quality care, and perform re-evaluations as needed.
• Maintain Accurate Client Records and Billing - Document services in the EMR, understand payer sources, and document supervisory visits with PTAs/HHAs.
