Job summary

Vivie is seeking a Physical Therapist for Home Health in Minnesota. You will evaluate clients, design personalized treatment plans, and guide therapeutic exercises to improve mobility and independence.

You will document care using the Vivie EMR system and collaborate with an interdisciplinary team to optimize discharge outcomes. You must hold a Minnesota PT license, maintain CPR certification, and have a valid driver’s license with access to a personal vehicle.

Qualifications

• Current Physical Therapist license in Minnesota.
• CPR certification required within 6 months of hire.
• Valid driver’s license and access to an insured personal vehicle.
• Ability to pass state mandated background check.
• Ability to read, write, and speak English proficiently.

Responsibilities

• Provide Direct Client Care - Evaluate clients and develop personalized treatment plans with short-term and long-term goals.
• Maintain accurate client records and billing using Vivie’s EMR system.
• Develop and oversee follow-up programs to support ongoing client success.
• Communicate effectively with clients, families, referral sources, physicians, and the interdisciplinary team.
• Perform duties as assigned beyond the listed responsibilities.

Schedule: PRN - Minimum 3 shifts per month - Monday to Friday - 8 a.m. to 4:30 p.m.

Pay range: $41.04/hour to $52.00/hour (hourly non-exempt) based on qualifications, experience, and location.

As a Physical Therapist (PT) in Home Health, you’ll be at the heart of our community, helping clients regain strength, mobility, and independence. Whether creating personalized treatment plans, guiding clients through therapeutic exercises, or collaborating with an interdisciplinary team, you’ll play a key role in enhancing the quality of life and overall well‑being of our clients.

As a Physical Therapist, You Will

• Provide Direct Client Care - Evaluate clients by obtaining and interpreting subjective and objective data. Develop personalized treatment plans with short-term and long-term goals in accordance with physician orders. Deliver high-quality care that empowers clients to achieve their functional goals. Perform re-evaluations as required to assess progress and adjust care plans accordingly.
• Maintain Accurate Client Records and Billing - Document all services accurately and promptly using Vivie’s EMR system. Demonstrate a thorough understanding of all payer sources and authorization requirements. Complete and document supervisory visits with PTAs and/or HHAs in compliance with regulatory guidelines.
• Develop and Support Follow‑Up Programs - Recommend and oversee continuation programs to support long‑term client’s success and optimize discharge outcomes. Make referrals to continuation programs as necessary to promote maximum independence and functionality.
• Communicate Effectively and Collaboratively - Maintain clear and compassionate communication with clients, families, referral sources, physicians, and the interdisciplinary team. Utilize exceptional verbal and written communication skills to ensure cohesive and effective care delivery.

This Job Also Requires

• Current Physical Therapist license in the state of Minnesota.
