Job summary

Vivie seeks a licensed Physical Therapist (PT) to provide hands-on home health care. You will assess clients, create tailored treatment plans, and collaborate with an interdisciplinary team to support recovery and independence.

This PRN role offers flexible scheduling, hourly pay ranging from $41.04 to $52.00, mileage reimbursement, and comprehensive benefits. You’ll join a supportive, patient-centered team delivering high-quality care.

Qualifications

• Must hold a current Minnesota Physical Therapist license.
• Excellent collaboration and teamwork abilities.
• Clear and compassionate communication with clients and staff.
• Ability to physically perform the duties of a PT.
• Successfully complete a state-m mandated background check and CPR certification within 6 months.

Responsibilities

• Provide Direct Client Care: evaluate clients, develop and adjust personalized treatment plans.
• Maintain accurate records and billing in Vivie’s EMR system.
• Develop and support follow-up programs to promote independence and discharge outcomes.
• Communicate effectively with clients, families, referral sources, physicians, and the interdisciplinary team.
• Carry out other duties as assigned.
