Job summary

Vivie is seeking a Physical Therapist (PT) in Home Health to deliver hands-on therapeutic support and personalized treatment plans. You will work with an interdisciplinary team to help clients regain independence and improve well‑being.

You will document services, navigate payer sources, and participate in supervisory visits with PTAs as part of a collaborative care approach. Minnesota licensure and CPR are required.

Qualifications

• Minnesota Physical Therapist license required.
• CPR certification within 6 months of hire.
• Ability to obtain and maintain state licensure and background check.

Responsibilities

• Provide Direct Client Care with personalized treatment plans and goals.
• Maintain accurate client records and billing using EMR system.
• Develop and support follow-up programs to optimize discharge outcomes.
• Communicate clearly with clients, families, and interdisciplinary team.
• Perform other duties as assigned.
