Job summary

Vivie is seeking a licensed Physical Therapist (PT) for Home Health in Minnesota. This PRN role offers flexible scheduling (minimum 3 shifts per month), with pay range $41.04–$52.00 per hour.

You'll deliver hands-on therapy, develop personalized treatment plans, and collaborate with an interdisciplinary team. You must maintain Minnesota PT license, have strong communication and teamwork, complete background check, and obtain CPR within 6 months.

Qualifications

• Current Minnesota Physical Therapist license.
• CPR certification within 6 months of hire.
• Strong collaboration and communication abilities.
• Ability to physically perform the duties required by the position.
• Background check clearance required.

Responsibilities

• Provide Direct Client Care - Evaluate clients and develop personalized treatment plans.
• Deliver high-quality care and adjust plans based on progress and physician orders.
• Maintain accurate client records and billing using the EMR system.
• Develop and support follow-up programs to promote independence.
• Collaborate with the interdisciplinary team and communicate clearly.
