Job summary

Vivie is seeking a Physical Therapist in Minnesota to provide direct client care in home health. You will evaluate clients, develop personalized treatment plans, and collaborate with an interdisciplinary team to improve mobility and independence.

You’ll maintain accurate records, handle billing, and participate in follow-up programs to optimize discharge outcomes. The role requires strong communication in English and a valid MN license, with driving to client sites.

Qualifications

• Active Physical Therapist license in Minnesota
• CPR certification within 6 months of hire
• Valid driver’s license and access to insured vehicle
• Ability to pass state background check
• Ability to read, write, and speak English proficiently

Responsibilities

• Provide Direct Client Care - Evaluate clients and develop personalized treatment plans with goals.
• Maintain Accurate Client Records and Billing using EMR.
• Develop and Support Follow-Up Programs to promote independence and discharge outcomes.
• Communicate effectively with clients, families, referral sources, physicians and the team.
• Other duties as assigned as part of the role.

Pay range: $41.04/hour to $52.00/hour (hourly non-exempt) based on qualifications, experience, and location.

As a Physical Therapist (PT) in Home Health, you’ll be at the heart of our community, helping clients regain strength, mobility, and independence. Whether creating personalized treatment plans, guiding clients through therapeutic exercises, or collaborating with an interdisciplinary team, you’ll play a key role in enhancing the quality of life and overall well-being of our clients.

As a Physical Therapist, You Will

• Provide Direct Client Care - Evaluate clients by obtaining and interpreting subjective and objective data. Develop personalized treatment plans with short-term and long-term goals in accordance with physician orders. Deliver high-quality care that empowers clients to achieve their functional goals. Perform re-evaluations as required to assess progress and adjust care plans accordingly.
• Maintain Accurate Client Records and Billing - Document all services accurately and promptly using Vivie’s EMR system. Demonstrate a thorough understanding of all payer sources and authorization requirements. Complete and document supervisory visits with PTAs and/or HHAs in compliance with regulatory guidelines.
• Develop and Support Follow-Up Programs - Recommend and oversee continuation programs to support long-term client’s success and optimize discharge outcomes. Make referrals to continuation programs as necessary to promote maximum independence and functionality.
• Communicate Effectively and Collaboratively - Maintain clear and compassionate communication with clients, families, referral sources, physicians, and the interdisciplinary team. Utilize exceptional verbal and written communication skills to ensure cohesive and effective care delivery.

This Job Also Requires

• Current Physical Therapist license in the state of Minnesota.
• CPR certification required within 6 months of hire.
