Qualifications

• Post baccalaureate degree in physical therapy and CAPTE approval.
• Current CPR certification and California licensure as a PT.
• Valid California driver’s license and automobile in good working order.

Responsibilities

• Provide PT services per physician's plan of care and state law.
• Assess patient status, mobility, strength and plan of care.
• Direct PT treatment and instruct patients and caregivers.
• Document progress summaries and communicate changes to physician.
• Supervise PT assistants and train home health staff as needed.
