Job summary

Bridge Home Health & Hospice seeks a licensed Physical Therapist to deliver therapy in patients’ homes under physician plans of care. You will assess mobility, strength, ROM, and functional status while coordinating with the care team to optimize outcomes.

You will instruct patients and caregivers on devices, document progress, and ensure compliance with state regulations. Supervision of PT assistants may be required.

Qualifications

• Strong communication skills, both written and verbal.
• Current CPR certification.
• Licensed driver with reliable vehicle as required by job duties.

Responsibilities

• Provide PT services per physician's plan of care and state regulations.
• Assess and document patient status, mobility, ROM, strength, and home environment.
• Direct therapy treatment and educate patients and caregivers on use of devices.
• Refer to other services and coordinate with care team as needed.
• Supervise PT assistants as per policy and state rules.
