Job summary

Bridge Home Health & Hospice in California is seeking a licensed physical therapist to provide skilled PT services in home health settings, following the physician's plan of care and applicable state law. You will assess patients, develop and implement plans of care, and educate families on therapeutic devices while coordinating with the care team.

Supervisory responsibilities include PT assistants and aides as required by policy.

Qualifications

• Post-baccalaureate degree in physical therapy (Doctor of Physical Therapy) approved by CAPTE.
• Licensed to practice as physical therapist by the Physical Therapy Board of California.
• Current CPR certification.
• Possesses and maintains valid California driver’s license and automobile.

Responsibilities

• Provide physical therapy services per physician's plan of care and state PT Act.
• Assess strength, mobility, gait, ROM; develop total plan of care.
• Instruct patients and families in use and care of therapeutic devices.
• Supervise PT assistants per organization policy and state regulations.
• Refer to other home health services as needed and prepare progress summaries.
