Job summary

Bridge Home Health & Hospice is seeking a licensed Physical Therapist to deliver patient-centered PT services under a physician’s plan of care within California’s Practice Act. You will evaluate functional status, direct therapy, and educate patients and families while coordinating with the care team.

You will supervise PT assistants, document progress, and participate in case conferences to ensure quality home health care consistent with organizational policies.

Qualifications

• Post-baccalaureate degree in physical therapy (DPT) approved by CAPTE.
• Licensed to practice in California as a physical therapist.
• CPR certification required.
• Valid California driver’s license; auto insurance required.

Responsibilities

• Provide PT services to patients per physician's plan of care and state practice act.
• Assess therapeutic/functional status and contribute to the plan of care.
• Direct physical therapy treatment and supervise PT assistants.
• Instruct patients and families in use and care of therapeutic devices.
• Refer to other home health services when appropriate.
• Prepare clinical/progress summaries and attend case conferences.
