Job summary

Nurse On Call Home Health, a part of LHC Group, is seeking a Physical Therapist to join our home health team. You will assess patient needs, develop treatment plans, and deliver therapies to improve mobility, function, and comfort in patients’ homes.

Responsibilities include evaluating functional status, coordinating with the care team, documenting progress, and ensuring care aligns with state regulations and plan of care. A valid PT license and CPR are required.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Valid driver’s license and access to a dependable vehicle or reliable transportation.
• Ability to function in any home setting regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Evaluates the patient’s functional status and physical therapy needs.
• Provides services within the scope of practice as defined by state laws, per the plan of care.
• Observes and reports patient response to treatment to supervising nurse/physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient and family regarding the plan of care, limitations, and progress.
• Visits patients according to Plan of Treatment and submits progress notes.
• Completes all PT evaluations and develops the plan of care per state guidelines.
• Reports outcomes of evaluation, goals, and anticipated care frequency.
