UnitedHealth Group seeks a dedicated Physical Therapist for Home Health in Olympia, WA. You will assess patient needs, develop and implement plans of care, and work with a care team to improve mobility and function in the home setting.

You will document progress, educate patients and families, and ensure compliance with state practice acts. A WA PT license and reliable transportation are required; CPR certification preferred.

Qualifications

• Current Physical Therapy licensure in the state of Washington.
• Current driver's license and access to a dependable vehicle, or reliable public transportation.
• CPR certification or ability to obtain within 90 days of hire.

Responsabilités

• Evaluates the patient's functional status and PT needs and assists the physician in the therapy plan of care.
• Provides services within the scope of practice defined by Washington state laws and in coordination with the health care team.
• Observes and reports patient responses to treatment and changes in condition to the supervising nurse or physician.
• Treats patients to relieve pain, restore function, and maintain maximum functioning.
• Instructs and informs the patient and family/caregivers about the plan of care and progress toward goals.
• Visits patients according to the Plan of Treatment and submits progress notes per policy.
• Completes PT evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
• Functions in diverse home settings regardless of demographics.
