Job summary

VitalCaring seeks a dedicated Home Health Physical Therapist to deliver patient-centered care in home settings across Texas. You will assess functional status, design and implement individualized plans, and work autonomously within the care team to restore mobility and independence.

You’ll educate patients and caregivers, monitor progress, and coordinate with nurses and physicians to ensure seamless transitions of care. A valid PT license and CPR certification are required.

Qualifications

• Graduated from an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• 1–2 years of clinical experience; home health preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• CPR certification current.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Document patient care and progress accurately in the EMR.
• Lead and supervise PTAs in accordance with state guidelines.
• Provide guidance to caregivers and support staff.
