Job summary

VitalCaring is hiring a Home Health Physical Therapist to deliver patient-centered care at patients' homes across Texas. You will evaluate functional status, design care plans, and provide hands-on therapy to restore mobility and independence.

This role emphasizes autonomy, collaboration with nurses and physicians, and compassionate, evidence-based care. You’ll educate families, document progress in the EMR, and help patients achieve meaningful outcomes in the comfort of their home.

Qualifications

• Graduate from an accredited Physical Therapy program (DPT or MS)
• Current PT license in the state of practice
• Minimum 1–2 years of home health experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Current CPR certification
• Valid driver’s license and reliable transportation

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential
• Develop and implement individualized care plans in collaboration with physicians and the care team
• Provide hands‑on therapy to improve mobility, strength, balance, and independence
• Educate patients and caregivers on treatment plans, exercises, and safety strategies
• Monitor patient progress and adjust treatment plans as needed; support discharge planning
• Document patient care and progress accurately in the EMR; participate in team meetings
