Job summary

VitalCaring Group in the United States is seeking a Home Health Physical Therapist to help patients recover at home, restore mobility, and maintain independence where they feel most comfortable. This highly autonomous, relationship-driven role partners with physicians, nurses, and caregivers to deliver patient-centered care, monitor progress, and document outcomes in the EMR.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of experience; home health preferred.
• CPR certification and valid driver's license required.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Document patient care and progress accurately in the EMR; participate in team meetings.
