Job summary

VitalCaring Group is seeking a Home Health Physical Therapist to deliver patient-centered care in patients’ homes, promoting independence and mobility. You will work autonomously with a collaborative care team and participate in ongoing clinical education.

We value compassionate clinicians, strong communication, and a commitment to high-quality home health outcomes. Qualified applicants will have a PT license, CPR, and a valid driver’s license, with 1–2 years of home health experience preferred.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience preferred.
• Current CPR certification and valid driver’s license.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans with physicians and care team.
• Provide hands-on therapy to improve mobility, strength, balance and independence.
• Educate patients and caregivers on treatment plans and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Document patient care and progress accurately in the EMR and participate in team meetings.
• Supervise PTAs per state guidelines and provide caregiver education.
