Job summary

VitalCaring seeks a Home Health Physical Therapist to help patients regain mobility and independence in the comfort of their homes. This highly autonomous, relationship-driven role focuses on delivering compassionate, patient-centered care and collaborating with the care team.

Ideal candidates hold a DPT or Master’s in Physical Therapy, an active PT license, CPR certification, and 1–2 years of home health experience. A valid driver’s license and reliable transportation are required.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master's).
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and patient education abilities.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and discharge planning.
• Identify additional patient needs and collaborate with interdisciplinary teams.
• Document patient care and progress accurately in the EMR.
• Participate in case conferences and team meetings.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines.
• Provide guidance and education to caregivers and support staff.
