Job summary

VitalCaring Group is hiring a Home Health Physical Therapist to deliver patient-centered care in patients’ homes. This role supports recovery, mobility, and independence with an autonomous practice style.

You will collaborate with physicians and the care team, supervise PTAs, and educate patients and families while building strong therapeutic relationships. The position emphasizes ongoing clinical education and growth.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience preferred.
• Current CPR certification.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans with physicians and the care team.
• Provide hands-on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Document patient care and progress accurately in the EMR.
• Participate in case conferences and team meetings.
• Supervise PTAs in accordance with state guidelines.
