Continuum Therapy Partners, LLC is seeking a dedicated Home Health Physical Therapist Assistant to support patient care in Milwaukee, WI and surrounding areas. You will work under a Physical Therapist to implement treatment plans and educate patients in home exercise programs, focusing on quality outcomes.

The role requires an Associate Degree in Physical Therapist Assistant and an active PTA license in good standing, with prior PTA experience preferred.

Qualifications

• Associate Degree in Physical Therapist Assistant required.
• Current, unencumbered state PTA license required.
• 1 year of PTA experience preferred.

Responsibilities

• Implements individualized treatment plans under the supervision of a Physical Therapist.
• Documents patient progress and reports changes in patient status to the supervising PT.
• Educates patients on home exercise programs and proper use of assistive devices.

Physical Therapist Assistant Requirements & Responsibilities

• Associate Degree in Physical Therapist Assistant Program required
• Current, unencumbered state Physical Therapist Assistant License
• 1 year of previous experience as a Physical Therapist Assistant preferred
• Implements individualized treatment plans under the supervision of a Physical Therapist
