Job summary

BAYADA Home Health Care in Manassas, VA is seeking a Home Health Physical Therapist to perform full-time home visits for adult and geriatric clients in senior living communities in the Manassas and Woodbridge area.

The PT will work as part of a multidisciplinary team, delivering rehabilitative care and skilled nursing at home, documenting progress and educating clients for safety and independent living.

Qualifications

• Current VA PT license.
• Graduation from an APTA-approved program with transcript or diploma.
• Ability to work independently with strong time management.
• Strong interpersonal and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Assess mobility, gait, strength, balance, and safety; document findings.
• Develop and adjust individualized PT plans and carry out interventions.
• Educate clients and families on rehabilitative care and safety.
• Document observations and interventions in EMR with accuracy.
