Job summary

BAYADA Home Health Care is seeking a Home Health Physical Therapist to provide skilled PT services through home visits in the Occoquan, Virginia area. This full-time role serves adult and geriatric clients in senior living communities, delivering compassionate 1:1 care in clients’ homes.

The ideal candidate holds a VA PT license, has experience with EMR, and can manage schedules independently while coordinating with a multidisciplinary team.

Qualifications

• Current VA Physical Therapist license in good standing.
• Graduation from an APTA- or AMA-approved program.
• Ability to work independently with good time management.
• Strong interpersonal, organizational and problem-solving skills.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements for mobility, balance, and safety.
• Develop and implement individualized care plans with exercises and gait training.
• Educate clients and families on rehabilitative care and safety at home.
• Document observations and interventions accurately in EMR.
