BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a full-time role providing in-home visits within the McLean, VA area. The PT will join a multidisciplinary team delivering rehabilitative care to adult and geriatric clients in senior living settings.

Responsibilities include performing functional assessments, creating individualized treatment plans, and educating clients and families.

Formación

• Current VA PT license.
• Minimum of one year of PT experience.
• Graduate from an APTA-approved PT program or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsabilidades

• Make home visits to clients in designated geographic territories.
• Perform mobility, gait, balance, strength and functional assessments.
• Implement wound prevention and repair interventions.
• Develop and revise individualized care plans with PT activities.
• Educate clients and families on rehabilitative care and safety at home.
• Document observations and interventions in an EMR via touch‑pad tablet.
