BAYADA Home Health Care, Inc. is seeking an experienced Physical Therapist (PT) for full-time outpatient home health visits in the Herndon, VA area.

The role emphasizes one-on-one patient care and advancing outpatient skills in a senior living office setting, servicing adult and geriatric clients on a per-visit basis. Qualified candidates will hold a VA PT license, have program accreditation, and demonstrate strong independent work and communication skills.

Qualifications

• VA license as a Physical Therapist.
• Graduation from a program approved by the APTA or AMA accreditation body.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem solving skills.
• Solid computer skills; EMR experience preferred.

Responsabilités

• Make outpatient visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements including mobility, transfer status, balance, gait, strength, endurance, and safety assessments.
• Develop and implement individualized care plans with manual therapy, gait training, balance, and related interventions.
• Continuously assess and revise the PT care plan and collaborate with nursing on the multidisciplinary plan.
• Educate clients and family members in rehabilitative care and safe home activities.
• Accurately document observations and interventions using the EMR/tablet system.
