Job summary

BAYADA Home Health Care, Inc. is seeking a Home Health Physical Therapist to perform outpatient visits in the Fairfax, Virginia area. The role is full-time and involves providing one-on-one PT services for adult and geriatric clients in senior living settings, billed on a per-visit basis.

You will conduct assessments, develop treatment plans, educate clients and families, document progress in an EMR, and collaborate with a multidisciplinary team to promote safe, independent living.

Qualifications

• Current Virginia PT license.
• Graduate from an APTA-approved program (or equivalent).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Experience with EMR is preferred.

Responsibilities

• Make outpatient visits to clients in designated geographic territories.
• Perform mobility, gait, strength, balance assessments and safety evaluations.
• Develop and implement individualized PT plans with exercises and gait training.
• Continually assess and revise the care plan and coordinate with nursing.
