BAYADA Home Health Care is seeking an experienced Home Health Physical Therapist for Woodbridge, VA. The role involves providing PT services in home health visits within senior living communities in the Woodbridge area.

Responsibilities include performing mobility assessments, wound care, and developing care plans, while educating clients and families and documenting care on a digital platform. Strong independent work ethic and VA licensure are required.

Qualificações

• Current VA PT license required and issued by state board.
• Graduation from a program approved by APTA or CAAHEA per transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.

Responsabilidades

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements for mobility and safety.
• Implement wound care interventions and evidence-based dressings.
• Develop and revise individualized care plans with PT and nursing.
• Educate clients and family members on rehabilitative care and activities.
• Document observations and interventions using a touch pad tablet.
