Job summary

BAYADA Home Health Care in Bristow, VA is seeking a Home Health Physical Therapist to provide full-time, in-home visits to adult and geriatric clients across Manassas and Woodbridge areas. The role emphasizes skilled rehabilitation and independence at home.

As part of a multidisciplinary team, you’ll deliver evidence-based PT services, document care, educate families, and participate in ongoing professional development within BAYADA’s supportive environment.

Qualifications

• Active VA Physical Therapist license.
• Graduation from an accredited Physical Therapy program.
• Ability to work independently and manage time effectively.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of mobility, transfer status, balance and gait.
• Implement wound care interventions using evidence-based dressings and topical agents.
• Develop and implement individualized care plans, including manual therapeutic exercises and gait training.
• Continually assess and revise the PT care plan with nursing in the multidisciplinary team.
• Educate and instruct clients and family in rehabilitative care and activities for health and independence.
• Accurately document observations and interventions using a state-of-the-art touchpad tablet.
