Qualifikationen

• LPN, LVN or RN license is required.
• Knowledge of governmental regulations, Medicare eligibility requirements, comprehensive understanding of potential care plan needs for the patient and coordination of necessary resources.
• Three years clinical experience is preferred.
• Travel within assigned geographical territory is required.

Aufgaben

• Work as part of our local sales team and report to a Market Executive of Sales
• Coordinate care from Hospitals and SNFs in the territory
• Meet with the patients at the bedside to discuss home health services, disciplines and what to expect at home.
• Support operations teams by ensuring they have the resources, information, and coordination needed to achieve a successful start of care.
• Assist with sales calls as needed to support Account Executive in addressing client needs and advancing business objectives.
