Job summary

Humana Inc. is seeking a Patient Care Coordinator in Iowa to join CenterWell Home Health.

The role requires an RN, LPN, or LVN license in Iowa and involves coordinating care with hospitals and SNFs, meeting patients at the bedside to discuss home health services, and supporting operations to start care. Travel within the assigned territory is required, and the position includes duties related to sales support and ensuring resources for a smooth start of care.

Qualifications

• Knowledge of governmental regulations, Medicare eligibility requirements, comprehensive understanding of potential care plan needs for the patient and coordination of necessary resources.
• Three years clinical experience is preferred.
• Travel within assigned geographical territory is required.

Required/Preferred Experience/Skills:

• LPN, LVN or RN license is required.
• Knowledge of governmental regulations, Medicare eligibility requirements, comprehensive understanding of potential care plan needs for the patient and coordination of necessary resources.

• Travel within assigned geographical territory is required.

This role is considered patient facing and is part of Human's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Scheduled Weekly Hours 40

Pay RangeThe compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $59,300 - $80,900 per year
