Job summary

Advocate Health is seeking a full-time Care Manager to support medically vulnerable patients. You will engage patients, educate on chronic disease self-management, and coordinate care across the care continuum to improve health outcomes.

Responsibilities include screening for social determinants of health, communicating with patients and caregivers, and using motivational interviewing techniques to support behavior change.

Responsibilities

• Monitor biometrics and medication adherence as part of chronic disease protocols.
