Job summary

Accompany Health is seeking a Health Advocate to support patients with complex health and social needs in their homes and via virtual visits. You will help build trust, coordinate care plans, and support clinicians across an interdisciplinary team.

Responsibilities include conducting in-home assessments, collecting vitals, facilitating video visits, assisting with social resources, documenting care plans, and coaching patients toward healthier, more connected lives.

Qualifications

• Experience in a patient-facing role (e.g., Medical Assistant, Community Health Worker, Health Coach, EMT).
• Comfortable working in patients’ homes and communities independently, including with medically or socially complex populations.
• Strong interpersonal skills and ability to communicate clearly with patients and clinicians.
• Basic clinical and technology proficiency (e.g., vital sign collection, using mobile apps, supporting video calls).
• Valid unrestricted driver’s license and access to an insured vehicle for daily use.

Responsibilities

• Build trusted relationships with patients to support high-need individuals.
• Facilitate virtual visits and ensure smooth technology use.
• Conduct in-home assessments and record vital signs and observations.
• Support behavioral health visits by collecting vitals and screenings.
• Navigate social needs and Insurance benefits for patients.
• Reinforce education and coach on medication adherence and healthy behaviors.
• Coordinate care logistics including referrals and tests scheduling.
• Deliver and support in-home services and supplies as needed.
• Document and communicate updates to the care team and accompany patients to visits.
