Job summary

Accompany Health is hiring Health Advocates to join our mission of delivering high-quality, in-home and virtual care for patients with complex needs. You will be the hands, eyes, and ears of the care team, building trust with patients, supporting virtual clinicians, and helping advance care plans.

This role involves visiting patients at home, taking vitals, and coordinating with a multidisciplinary team to improve health outcomes.

Qualifications

• Experience in a patient-facing role (e.g., Medical Assistant, Community Health Worker, Health Coach, EMT)
• Comfortable working in patients’ homes and communities independently
• Strong interpersonal skills to build rapport with patients and clinicians
• Valid unrestricted driver's license and access to an insured vehicle for daily use

Responsibilities

• Build trusted relationships with patients to support home care
• Facilitate virtual visits and assist with technology setup
• Record vitals and document in the EHR during home visits
• Support care plan adherence and health coaching
• Coordinate care logistics including referrals and appointments
• Deliver and support in-home services and supplies
• Document communications for the care team
