CenterWell Home Health is seeking a motivated Occupational Therapist Assistant to deliver home-based therapy under the supervision of an OT. You will implement treatment plans, adapt activities, and educate patients and families in home programs.

This role requires OTA licensure, CPR, a valid driver's license, and preference for 6 months of home health experience. Competitive pay and a comprehensive benefits package are offered.

Qualificações

• Current and unrestricted OTA licensure.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsabilidades

• Provide therapy services planned, delegated and supervised by the qualified Occupational Therapist.
• Assist in implementing programs to restore and improve activity performance.
• Design/adapt equipment and environments to improve function and independence.
• Deliver therapeutic treatment and instruction per physician orders to improve strength, ROM and coordination.
• Document patient progress and prepare reports per company requirements.
• Instruct patients and families on home exercise programs and use of adaptive equipment.
• Participate in care coordination and discharge planning with the interdisciplinary team.
• Coordinate community reintegration activities to aid physical reconditioning and psychological adjustment.
