Job summary

CenterWell Home Health is seeking a Home Health Occupational Therapist Assistant to join our patient-focused team. You will provide therapy services, under supervision by an Occupational Therapist, helping patients restore independence in their homes.

Responsibilities include implementing OT plans, adapting equipment and environments, and educating patients and families on home programs. A current OTA license, CPR, and a valid driver's license with insurance are required.

Qualifications

• Current OTA licensure unrestrictio​n in your state.
• CPR certification required.
• Valid driver's license with auto insurance and reliable transportation.

Responsibilities

• Provide therapy services planned, delegated and supervised by the Occupational Therapist.
• Assist in implementing vocational/education programs as designed by the OT.
• Design/adapt equipment and living environments to improve function.
• Provide treatment to improve/restore strength, coordination and range of motion.
• Document patient progress and maintain records as required by the Company.
• Instruct patients and families on home programs and use of adaptive equipment.
• Participate in care coordination and discharge planning with the interdisciplinary team.
• Design community reintegration activities to assist physical reconditioning and psychological adjustment.
